Social determinants of health (SDOH) play a critical role in shaping outcomes for patients with heart failure. This quality improvement project explores nurses’ perspectives and potential biases toward these patients, aiming to assess whether a targeted educational seminar can enhance nurses’ understanding of heart failure management while simultaneously addressing SDOH. The intervention includes pre- and post-surveys to measure changes in knowledge and confidence. By equipping nurses with evidence-based strategies and clinical guidelines, the project seeks to strengthen patient-provider relationships, improve patient adherence to treatment plans, and reduce 30-day readmission rates. Anticipated outcomes include improved posttest scores and increased nurse capacity to recognize and respond to SDOH barriers, ultimately promoting more safe, equitable, and effective care.
